Required documents for an application
(Original documents are necessary)

For applicants born in Quebec:

• Birth certificate from the civil registry with the names of the parents
• Quebec health insurance card
• Transcript of marks from the MEQ

For applicants born outside of Quebec:

• Citizenship card or permanent resident card or IMM1000 form
• Quebec health insurance card
• Transcript of marks from the MEQ if the studies were done here. If not, you need the recognition of the studies done abroad.
• Quebec selection certificate.
